Dear Parent/Player,

The Spring Soccer Picture Day will take place at Rainbow Park on Saturday, April 26th.

The exact time-schedules for the individual teams will be made available online after April 15th, please visit either www.ayso64.org or www.lzphoto.com for the details.

All registered players are entitled to a Basic package, paid by Region 64, which includes one Digital Memory Mate featuring the Team picture along with the Image of the Player and one Magnet. (Additional packages or a la Carte can be ordered by specifying these in Section #2. Prices already include the Sales Tax, please include the exact payment in the envelope for the extra orders).

If you can’t make Picture Day, please notify your Coach / Team Manager ASAP and contact the company to reschedule your appointment to the Make-Up Picture Day at Creekside Park in Cupertino on Sunday, May 4th. Individuals making use of the Make-Up Picture Day MUST pick up their pictures on Thursday, May 22nd at Queensborough, 1138 Miller Ave. in San Jose.

Please use “Black Ink” to fill in the Ordering Forms. Each player must submit his/her form complete with both player and team information. The player info should include the Player’s Name, Address and Phone number while the team info should include the Team Name, the Team Number, the Region and the name of the Head Coach. Please bring the completed form to the Picture day.
